From nursing home to community living…going home again.  Nursing Facility Transition assists individuals whose care is paid for through the Medicaid Program to leave the nursing facility and move back into community-based living.  Assistance is provided through:

• Meeting to develop a person-centered plan with goals and objectives
  
• Locating/modifying housing
  
• Providing some financial assistance for essentials
  
• Identifying community resources
  
• Six months follow up after the transition
