NURSING FACILITY TRANSITION

The goal of nursing facility transition services is to provide nursing facility residents with the opportunity to look at home and community-based living options. These services are available to residents of nursing facilities who are Medicaid eligible and would like to return to their own home, move in with family members, find an apartment, or choose another option.

Our staff is available to help throughout the transition process. It begins with an interview and development of a person centered plan. We are experts in locating housing and in adaptation of your current home to meet your needs. Financial assistance is available for some one-time moving expenses such as a security or utility deposit, housekeeping supplies, and essential furniture. Every effort is made to identify community resources to address the individual’s needs once the transition is completed.

HOME ASSESSMENT
Our goal in providing a home assessment is to help you stay in your homes as your level of independence changes. Our Registered Occupational Therapist is available for a home assessment to evaluate your needs at home as your situation changes. There are many modifications that can be made at reasonable cost to help you live safely and independently in your home. You may benefit from meeting with an options counselor from West Michigan Long Term Care Connections to assist you with decisions regarding long-term care or related services.
