Nursing Home Transition
For most Nursing Home residents what they receive is exactly what they need—round-the-clock care. Others are in a nursing home due to a temporary situation like rehabilitation. Often there comes a time that those residents would like to transition back into independent living. But that transition can be full of obstacles that make it difficult for a resident.
Our NFT Program empowers older adults to transition out of nursing facilities into community-based living options that offer the freedom they are seeking. We provide individuals with personalized assistance as they transition from a nursing home or other long-term care setting into independent community-based living arrangements including assisted living, personal care homes, adult family homes, shared housing, or private residences.

What is Nursing Facility Transition (NFT)?
Our Nursing Facility Transition program helps individuals living in nursing homes transition into independent community-based living. We can help regardless of age (18+) or disability. We use a person-centered planning approach to assist people with returning to their own homes, moving in with family/friends, or finding new housing.
Nursing facility residents may request transition services for themselves, or referrals may be made by a family member, friend, or nursing facility staff.
Once a referral has been made, a transition interview will be conducted to obtain the information necessary to assist with long-term care goals.
